1. DOWNLOAD UPDATED ORDER FORM HERE - since prices and availability may vary please download a new and current order form EACH time you order.
2. When prompted select "SAVE".
3. Open the saved file ... the order form is a Microsoft Word document.
4. Indicate in the right hand column how many of each item you want.
5. After filling in your order amounts "Save" the order form again.
6. Email the order form to: orders@mypetcarnivore.com
7. We collect payment for food orders by cash or check at the time you receive it. Shipped supplements and treats will be billed by Paypal after the shipping cost is determined and the total amount is approved by you.

DELIVERIES: Unfortunately, we must add a charge of $10 per order for delivery ($20 for Chicago home delivery) to cover the sharply rising gas prices. Thanks for your support!
